JOB DESCRIPTION
Requirements:
- Master's Degree in Social Work from a professional school accredited by the Council on Social Work Education.

- Current state license, L.S.W. (Licensed Social Work) or L.C.S.W. (Licensed Clinical Social Work).

- Minimum of one to two years hospital discharge planning experience.

- Experience in working with a diverse population with a wide range of problems (i.E., death and dying, maternal and child care, substance abuse, mental health, disability, chronic illness, etc.).

To provide case management & milieu management, complete psychosocial assessments, assist in the development and re-evaluation of Treatment Plans and work as a member of an interdisciplinary team. In collaboration with Case Manager develop and implement discharge plans, coordinate referrals and authorizations. 
Responsibilities:
Responsible for direct data collection and assessment of the patient's condition and stability based on patient's status including psychosocial assessments.

Identifies biophysical, psychosocial environment, self-care, educational, spiritual and discharge needs of the patient as evidenced in the patient's medical record.

Responsible for the initial assessment and ongoing reassessment of the patient's condition, stability and determination of psychosocial needs.

Collaborates with other professional disciplines to plan, implement, evaluate and revise the Plan of Care for each patient, as evidenced in the patient's medical record. The Plan of Care is discussed with and developed as a result of coordination with the patient/family/significant others when appropriate.

Responsible for management of patient throughout stay, including discharge planning and effectively coordinates with others involved in the patient's treatment and aftercare plan.

Assesses and reports domestic violence, elder, dependent adult and child abuse in accordance with regulatory standards and hospital policy and procedures.

Bases decisions on facts, clinical reasoning and sound judgment seeks assistance when necessary.

Responsible for the evaluation of the patient's response to/ effectiveness of the implementation of the Plan of Care.

Documents all significant contacts and services provided in the medical record.

Coordinates safe discharge plans and referrals for continuation of care.

Provides Alcohol and Drug Screening and brief intervention as appropriate, makes a referral to community resources if appropriate and patient is agreeable. Documents need for treatment and patient response in the medical record.

Notifies Manager/Charge Nurse/Director/House Supervisor, immediately of significant problems or unusual incidents and when appropriate, takes actions to intervene.

Provides crisis intervention and supportive assistance to patients/families as appropriate.

Conducts support meetings for patients/families as indicated.
