Requirements:
The Clinical Manager is responsible for ensuring that patient care is coordinated and managed appropriately. Responsible for coordinating home care which includes establishing and maintaining positive relationships with customers, making patient and personnel assignments, coordinating patient care, coordinating referrals, assuring that patient needs are continually assessed, assuring the development, implementation and updates of the individualized plan of care, as well as the supervision of clinical personnel.
Responsibilities:
- The Clinical Manager (Registered Nurse) (RN) coordinates nursing services, supervises staff, and coordinates home care services/delivery in conjunction with third- Arty payor sources and services as a resource to the clinical team for necessary prior authorization and reporting requirements
- Provides leadership and supervisions to all nursing staff supervised and conducts and coordinates case conferences, in- Ervice programs and team meetings as requested
- Responsible for the efficient assignment, scheduling, and supervision of staff and on- Call coverage and maintains records of case assignments and management forms as requested
- Organizes and directs patient care conferences and works effectively with physicians and other members of the home health team to promote coordination of patient services, positive patient outcomes, and effective documentation
- Reviews medical records routinely and conducts quarterly clinical record reviews to supervise the quality and timeliness of care and documentation
- Investigates adverse events to identify opportunities to improve care and follow- Ups as requested
- Knowledge of the Kinnser EMR System
- QA for all disciplines
- Chart audit for compliance including frequencies, appropriate orders, 485s and F2F
- Answer urgent Clinical questions in a timely manner
- Perform Oasis Analysis;create POC;export to IQies
